Wearing off diary

To help follow your symptoms and response to treatment, record your daily observations in the diary pages (see examples of some symptoms, given below). This record is extremely useful when taken along to medical appointments, allowing the doctor to follow how the medication is working and, as a result, to prescribe the therapies most appropriate and effective for your particular needs.

If you need more pages for your diary, make photocopies from the diary master card.
