American Parkinson Disease Association
The American Parkinson Disease
Association (APDA) has launched a new
website, www.youngparkinsons.org,
specifically targeted to the issues and concerns
of the younger patient population (ages 21-50).

PD
MED: A Phase III Parkinson's Disease Trial
Birmingham Clinical Trials Unit (BCTU)
The PD
MED Study is a large, simple,
"real-life" trial that aims to determine
much more reliably which class of drugs provides
the most effective control, with the fewest side-effects,
for both early and later PD.
Patients with early PD are randomised between
DA, MAOBI and LD alone, with the option to omit
either the MAOBI or LD alone arm. Those with
later PD are randomised between COMTI, DA and
MAOBI, with the option to omit either the DA
or the MAOBI arm. The main outcome measure is
patient-rated quality of life, using the PDQ-39
scale, which assesses all aspects of the patient’s
life, and is sensitive to changes considered
important to patients but not identified by clinical
ratings.
The study is funded by a grant from the NHS
Health Technology Assessment Programme and
is supported by the European Parkinson's Disease
Association (EPDA), the Parkinson's
Disease Society (PDS) and the Parkinson's
Disease Nurse Specialist Association (PDNS). |

Many people with PD have difficulty initiating
and sustaining walking in conditions which would
normally present no problems (such as an unobstructed
corridor). Indipendent I Go or INDIGO
exploits a little known effect in Parkinson’s
Disease (PD) called kinesia paradoxa by displaying
moving visual cues in the user’s peripheral
visual field within specially adapted glasses.
These visual “obstructions” help
a small proportion of people with PD (effective
over 11% of people with PD in intermediate stages
according to the developers of Indigo) undergo
a dramatic release from these symptoms.
INDIGO is the new commercial name (launched
on June, 1st, 2005) of Parkwalker.
PARKWALKER has been developed from the results
of an earlier EU R&D project called “PARREHA”
(IST-1999-12552, 2000-2003). The PARREHA project
demonstrated that the symptoms of PD where sufferers
developed difficulty initiating and sustaining
walking in conditions which would normally present
no problems (such as an unobstructed corridor),
could be alleviated in suitable people with PD
through a simple to use, wearable aid, ParkWalker.
By targeting specific effects of PD, ParkWalker
acts as a mobility aid, using tailored visual
stimulation to dramatically increase mobility
in suitable users. PARREHA found that this effect
was dramatic and amounted to some subjects being
able to walk and talk normally where before they
displayed considerable disabilities. Small scale
trials suggested the PARREHA prototype device
was effective, acceptable in principle to users
and had no side effects.
In May 2003 the PARREHA consortium formed ParkAid,
a start-up company whose sole aim is to exploit
ParkWalker and build a supporting service around
it (ParkService). ParkAid also developed ParkWalker
from the PARREHA prototypes – concentrating
on ease-of-use and comfort. ParkWalker, which
took the commercial name of INDIGO (INDependent
IGO), is based upon a small display which clips
on to a normal pair of glasses. Visual stimulation
is generated by a small portable device which
can also wirelessly connect to a clinician when
the user is at home.

Mouse-Cage is
a software application that helps people with
hand tremor control their computer mouse.
Mouse-Cage was developed for people with Essential
Tremor (ET), Parkinson's Disease, Multiple Sclerosis
(MS) and other conditions that may cause hand
shake or tremor.
The application features an unusual dual-cursor.
A special guide cursor provides visual feedback
by showing the user’s actual hand movements,
while the normal Windows cursor takes an average
of these movements and predicts where the user
intends to click.
Another innovative feature of MouseCage is its
‘cage’ key. When users are near their
intended position for clicking, they can ‘cage’
their mouse by pressing a user-defined key on
their keyboard or mouse. When caging is activated,
overall mouse movement is drastically reduced
to allow fine control. As a further refinement,
if the cage key is pressed a second time, cursor
movement stops altogether so that users can click
in their own time. According to Nick Allen, co-developer:
“The cage key allows you to take tight
control of your mouse on demand, and our trials
have shown that people find the caging concept
both intuitive and empowering. There is a degree
of satisfaction in hitting that cage key and
finally getting the mouse to slow down.”
While some conditions result in an almost constant
tremor, there are also intermittent types. MouseCage
deals with this by employing a tremor-detection
feature that switches the computer between normal
operation and MouseCage driven operation depending
on the operator’s pattern of hand movements.
Nico Cuppen explained: “Even with MouseCage
running, a person without tremor can use the
mouse virtually as normal. Because the cage key
and other functions are also designed to work
in non-tremor mode, all users can take advantage
of these features if they need a little more
control.”
More information can be found on the Mouse-Cage
website www.mousecage.org |

Health
Advice for Travellers
Travel across the world is now so common that
it is easy to forget the health risks which can
be involved and the fact that very few countries
offer such easy access to medical facilities
as Britain. This area of the UK Department of
Health website contains advice for travellers
about planning ahead, staying healthy and getting
treatment elsewhere in the world. Also in this
area is information about the E111, UK citizens’
passport to free or reduced-cost emergency care
in most other European countries. |
